In a pioneering example of joined-up community healthcare, five of West London NHS Trust’s teams united for a Musculoskeletal (MSK) Community Health Day earlier this month, offering residents health checks and practical wellbeing support.
The event saw teams from the Diabetes Service, Healthy Ealing Team (HET), Ealing Smokefree service, Tuberculosis, and the Diabetes Health & Wellness Coaching Programme join forces. Their shared goal was to improve health outcomes by tackling prevention, independence, and patient engagement head-on.
Prachi Soni, Service Manager for Long Term Conditions said: “This was integrated care at its best! We connected with residents in the community in a meaningful way and provided support that hopefully makes a real difference.”
Residents benefited from:
• Interactive diabetes education: “Sugar Games” illustrated carb impacts on blood sugar, complemented by multilingual guidance in English, Punjabi, and Tamil.
• Public health messaging: HET led early detection campaigns, covering stroke, cancer and Alzheimer’s awareness, alongside harm reduction initiatives with Recovery Intervention Services (RISE).
• Personalised Quit Support: Ealing Smokefree service delivered its eight-week cessation programme, featuring behavioural coaching, Nicotine Replacement Therapy and vaping options.
• Lifestyle Coaching: Residents explored information about a 12-week programme with practical wellness planning and community cooking classes at the University of West London.
• Tuberculosis Awareness: The TB team invited attendees for free testing and open conversations to address stigma and increase better understanding.
The event marked another win for community-led, joined-up healthcare across Ealing, helping more people to live a healthier and more independent life in the borough.
